Numbness, needle like tingling and pain in right hand?

I have had carpal tunnel release surgery in February. I now have all the numbness, tingling and pain again but it goes up my arm to the shoulder. The symptons are always there but at times are so unbarable. My hand doc said it's not from carpel tunnel because i have the symptons in my pinky finger also, which is a nerve that is not connected to the rest. Any possibilities of what it could be?
