How Should I Set Up This Reading Group?

I don't know how exactly, but i remember in my 2nd/3rd combination class my teacher did a good job. we were split by our level, not our age, and that was great because everyone in your group read at the same pace and stuff. but i don't know how she found out our reading levels and what books she gave us
